Revised Lifeline Certification Form, Brochure and Advertisement Available

ITA has been working for several weeks with the IUB to update the Iowa Lifeline brochure and Iowa Lifeline Assistance Certification Form for use by ITA members.   

Please destroy all current copies of brochures and certification forms.   

The new documents (with the May 2012 revision dates) have been changed substantially in order to comply with the FCC’s Order on Lifeline and LinkUp in February.  Both documents have grown from one page each to two pages each.  Customers are required to provide new and additional information, along with a long list of new certifications.   

The new Lifeline Certification form can be used for new applicants and for re-certifying existing customers.  To access an electronic version of the new Certification Form go to the IUB website athttp://www.state.ia.us/government/com/util/consumer_information/lifeline.html.  

New Lifeline Applicants.  ITA advises all members to immediately begin using the new certification form to certify new Lifeline customers.   

Re-certifying Existing Lifeline Customers.  For existing Lifeline customers, after talking with the IUB about this, we advise members to send the new certification form to all current Lifeline subscribers immediately after June 1st, and give customers 30 days to return their re-certifications.  The revised FCC rules set a December 2012 deadline for ETCs to "recertify” all Lifeline customers who were certified prior to June 1st, therefore, we suggest you hold off on sending the new certification forms until after June 1st to avoid having to "re-certify” your customers again in December.  

Advertisements.  Many ITA members publish Lifeline advertisements around this time of the year.  We have updated the ITA template that you may use as a basis for your ads, and it is available here.
